Mō te tūranga - About the role

The Home team offers a seven day a week service from 8am to 8pm. The focus of the service is hospital avoidance and supported discharge working across a range of settings including the community, Emergency Department, Internal Medicine Assessment Unit, and the in-patient wards.

  • Support the early discharge of appropriate patients from Dunedin Public Hospital alongside a multi-disciplinary team

  • Monitor patient recovery and ensure everyone receives appropriate rehabilitation and/or packages of care at the right time to maximise their functional ability and maintain their independence in their own home

  • Provide a responsive community-based service for patients, who (while being medically stable) require urgent assessment and immediate coordination of services to prevent Emergency Department presentations or hospital admission

  • Fantastic opportunity for an Allied Health Professional who is adaptive to change, resourceful, flexible to service needs and future development and has a genuine passion for working in the community

Mōu ake - About you:

  • Proven ability to adapt to change and be flexible within the working environment with a desire to create change for the better of our service users

  • Advanced comprehensive clinical assessment and problem-solving skills

  • Ability to direct and delegate to Rehabilitation Assistants.

  • Excellent communication skills to liaise with health professionals, both within the in-patient services and in the community setting

  • Recent experience of working with the older adults in acute, rehabilitation or community settings

  • Be a Registered Health Professional with a current APC, a minimum of two years post registration in an appropriate setting and have a full clean NZ driver's licence

Utu whānui - Salary range

This role belongs to the DHBs/PSA Allied, Public Health and Technical Multi-Employer Collective Agreement and falls between Step 1 ($59,035 p.a) and Step 8 ($85,992 p.a pro rata) of the Degree-based Allied, Public Health and Technical Salary Scale.
